If you recover from a mental health issue, you want to prevent it from ever coming back. Characteristic of psychological problems is that it does not arise over night. Often it goes gradually and in such a way that you only notice it when you had a relapse. This theme fits in this situation. By making a relapse prevention plan you describe four different phases, where green stands for everything is fine, yellow for feeling slightly less good, orange for significantly less good and red for the actual relapse. You will go through each phase for yourself and note down what you, or your environment, can do for you if you are in a particular phase. This way it is possible to intervene early and prevent a relapse. If you nevertheless have a relapse, you can use the advice on how to pick up the thread again.
